Job Opportunity: RN Care Coordinator - Part or Full time

Positions Available: 1 Part time or Full Time
Hours: Monday – Friday; 8am - 5pm   No nights or Weekends!

Our Cook Medical clinic is seeking a full or part time Registered Nurse Care Coordinator to join our team. The RN Care Coordinator works in collaboration and continuous partnership with chronically ill or “high- risk” patients and their family/caregiver(s), clinic/hospital/specialty providers and staff, and community resources. This position requires advanced clinical nursing skills to use discretion and independence while coordinating care. The RN Care Coordinator will have both face to face and indirect contact with patients of all ages.
 
RN Care Coordinator Responsibilities: 
  • Serve as contact point, patient advocate, and informational resource for patient’s care team, family/caregiver(s), payers, and community resources
  • Cultivate and support primary care and specialty provider co-management with timely communication, inquiry, follow-up, and integration of information into the care plan regarding transitions-in-care and referrals
  • Assist with the identification of “high-risk” patients (the chronically ill and those with special health care needs), and add these to the patient registry (or flag in EMR)
  • Provides mentoring/coaching of other population health and care coordination team members and coach patients/families toward successful self-management of their chronic disease
  • Utilize tools and documents that support a guided care process, collaborate with patients/family toward an effective plan of care
  • Assess patient and family’s unmet health and social needs
  • Provide effective communication to improve health literacy
  • Develop a care plan based on mutual goals with patient, family, and provider’s emergency plan, medical summary, and ongoing action plan, as appropriate
  • Monitor patients adherence to plan of care and progress toward goals in a timely fashion, facilitate changes as needed
  • Create ongoing processes for patients/families to determine and request the level of care coordination team support desire over time
  • Perform triage of patients presenting to facility or by phone as needed
  • Assist support staff with patient inquiries directly or in-directly utilizing phone, patient portal and EMR system
  • Ensure effective tracking of test results, medication management, and adherence to follow-up appointments 
 
RN Care Coordinator Requirements:

  • Associate Degree in Registered Nursing with an active MN license
  • EMR or EHR experience
  • Three years of clinical experience working with chronic disease patients or related case management experience, five or more years preferred
